feat(A34-A38): Knowledge base pipeline — ChunkSplitter, OCR, DocGen, KBManager

- A34: ChunkSplitter — 4 strategies (paragraph/heading/fixed/sentence)
- A35: OCRTool — PaddleOCR > Tesseract > bash fallback
- A36: DocGen — Markdown→HTML/PDF/Word (weasyprint/pandoc/python-docx)
- A37: KBManager — create/add/search/list/delete/info knowledge bases
  Persistent storage in ~/.lambdagent/knowledge_bases/
  Auto-chunking on add, SimpleVectorStore for retrieval
- A38: 15 integration tests all passing
- Registry now has 28 built-in tools
